Fractional CFO services for medical practices

A fractional CFO gives a practice senior financial oversight without a full-time salary. For medical practices that means monthly financial reporting you can act on, profitability broken out by provider and service line, payer mix analysis, cash flow forecasting, budget construction, and the numbers behind decisions like hiring a provider, adding a location, or buying equipment.

Engagements run monthly, typically $2,500–$8,000 by practice size and scope. I am not a CPA. Audits, reviews, compilations, and tax filings are outside my scope and are engaged directly by the client with a licensed CPA of their choosing.

What you get monthly

  • Financial statements with written commentary explaining what changed and why
  • Profitability by provider, location, and service line
  • Payer mix with trend and margin by payer
  • Days in A/R, net collection rate, denial rate
  • Rolling 13-week cash forecast
  • Budget versus actual with variance explanation
  • A monthly call, not just a PDF

The decisions this is built for

  • Can we afford another provider, and when do they break even?
  • Is this service line profitable or is something else subsidizing it?
  • Should we renegotiate or drop this payer contract?
  • Buy or lease?
  • What is this practice worth, and what would a buyer scrutinize?

What it costs

$2,500–$8,000 monthly. Scope and size drive the range.

Who this is not for

Practices that need a full-time CFO or that are not yet at a scale where financial reporting drives decisions typically under $750,000 in annual revenue.

State rules

Financial reporting is delivered remotely nationwide. State-specific considerations apply where NP scope of practice affects service-line profitability and billing structure.

Start with an assessment, not a contract

Two weeks, fixed fee, $2,500 to $5,000 depending on practice size. You get a written finding: where the money is going, what each leak is worth annually, and what it costs to fix. You own that report whether or not you hire me for anything after it.
